Yes, long standing issue. The # in the URL breaks the phpBB code, it seems. Upgrading to a less than ancient version would probably solve the problem but I'd be surprised if that happened.

You need to use the BBCode version of the tags for Bulletin Boards like this.

No. :wink: First host the image at any of the many free hosting sites and then use the image tags to link it into a post, most of which will automatically generate them for you. You can 'attach' images using the Img tags but I don't think it would help much. In your shoes I would be contacting support.
